We know you have many choices when choosing a dentist; so we have office hours to accommodate your family's busy lifestyle. We are open Monday through Thursday from 9 am to 8 pm; as well as staff to answer your phone calls from 8 am and on Fridays. If for any reason you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible. We offer second opinions at no charge - simply have current x-rays emailed to [email protected].
Insurance & Billing

Proven Dental is a Preferred Provider for United Healthcare, Guardian, Cigna, Wisconsin Dual Complete, and a few other PPO plans. We will be happy to file your insurance claims for you. Proven Dental accepts most traditional insurance plans, but please contact our office to verify acceptance of your plan. We also accept checks, cash or credit cards for balances not covered by your insurance. We request co-payments and un-reimbursed services are paid at the time of service.
