Insurance and Billing
Proven Dental accepts most traditional insurance plans, but please contact our office to verify acceptance of your plan. Proven Dental is a Preferred Provider for Delta Dental, United Healthcare, Guardian, Cigna, and other PPO plans.  We will be happy to file your insurance claims for you. We also accept checks, cash or credit cards for balances not covered by your insurance.  We request co-payments and un-reimbursed services be paid at the time of service.
